Positions available are:
REGISTERED NURSES
Must have the appropriate state license. Experience in home health preferred, but we are always willing to train. Ability to assess and evaluate client needs, communicate with physicians, family, client and other staff members. Responsible for case management and supervision of LPN’s and CNA’s who may also be caring for the client. Must be a licensed driver or have adequate transportation to allow for travel to client locations.
LICENSED PRACTICAL NURSES
Must have the appropriate state license. Experience in home health preferred, but we are always willing to train. Ability to implement the plan of care as initiated by the Case Manager. Observe client for changes and communicate with other team members and the physician when necessary. Must be a licensed driver or have adequate transportation to allow for travel to client locations.
PHYSICAL THERAPISTS
Must have the appropriate state license. Experience in providing PT services in the home preferred, but not required. Must be a licensed driver or have adequate transportation to allow for travel to client locations.
OCCUPATIONAL THERAPISTS
Must have the appropriate state license. Experience in providing OT services in the home preferred, but not required. Must be a licensed driver or have adequate transportation to allow for travel to client locations.
CERTIFIED NURSING ASSISTANTS
Must have the appropriate state certification. Experience in home health preferred, but we are always willing to train. Ability to implement the plan of care as initiated by the Case Manager. Observe client for changes and communicate with other team members and the physician when necessary. Must be a licensed driver or have adequate transportation to allow for travel to client locations.
